A prasugrel (prasugrel hydrochloride) coupon from Rx.com lets you pay a discounted cash price for this generic antiplatelet tablet, the same medicine sold under the brand name Effient. Prasugrel is a P2Y12 platelet inhibitor prescribed after a heart stent to lower the risk of blood clots, and because it is available as a generic it is already one of the lower-cost options in its class. Even so, the cash price for the exact same tablet can vary a lot from one pharmacy to the next, so it is worth comparing before you fill. What is prasugrel and how does it work?
Prasugrel is a prescription antiplatelet medicine in the thienopyridine class, known as a P2Y12 (ADP receptor) inhibitor. It works by blocking a receptor on platelets so they are less able to clump together and form a clot. It is FDA-approved to reduce the rate of thrombotic cardiovascular events, including stent thrombosis, in people with acute coronary syndrome (ACS) who are being treated with percutaneous coronary intervention (PCI) — that is, people with unstable angina or an NSTEMI, and people with a STEMI treated with primary or delayed PCI.
Prasugrel is usually started with a loading dose at the time of the stent procedure, then continued as a once-daily maintenance tablet taken along with low-dose aspirin. It is not indicated for ACS that is managed with medication alone (no PCI), and it is not a first-line drug for stroke prevention. Your cardiologist decides the dose and how long you stay on it.
How much does generic prasugrel cost without insurance?
Generic prasugrel is widely available in the US, and generics are typically far cheaper than the brand they copy. That does not mean every pharmacy charges the same thing. Cash prices for an identical 30-day supply of the same strength can differ substantially between a big chain, a grocery store pharmacy, and an independent down the street, because each sets its own cash price. If you are paying out of pocket, or if your plan's deductible has not been met, the pharmacy you choose can matter as much as the drug you are prescribed.
Rx.com compares cash prices across more than 60,000 pharmacies nationwide so you can see who is cheapest near you before you hand over your prescription. There is no manufacturer savings card for generic prasugrel, so a discount coupon is the practical way to lower the cash price. Enter your ZIP code above to see today's price, then show the coupon at the pharmacy counter. It is free to use, there is no membership, and you can use it whether or not you have insurance — just ask the pharmacist to run the coupon price and compare it to your copay.
Prasugrel vs. clopidogrel, ticagrelor, and other alternatives
Prasugrel is one of several P2Y12 inhibitors used after a stent. The main alternatives are:
- Clopidogrel (brand Plavix) — the oldest and most widely used option, taken once daily. It has to be activated by the liver enzyme CYP2C19, so people who are poor metabolizers of CYP2C19 may get less benefit from it. Prasugrel's activation is not meaningfully affected by CYP2C19 status, which is one reason it is sometimes chosen for people with clopidogrel resistance who are having PCI.
- Ticagrelor (brand Brilinta) — a different type of P2Y12 inhibitor, taken twice daily.
- Effient — the original brand name for prasugrel; the generic contains the same active ingredient.
All of these are stronger blood thinners than aspirin alone and all raise bleeding risk. Which one is right depends on your procedure, your age, your weight, and your bleeding history — this is a decision for your cardiologist, not something to switch on your own.
Safety information you should know
Prasugrel carries an FDA boxed warning for bleeding risk. It can cause significant and sometimes fatal bleeding. It should not be used in people with active pathological bleeding or a history of stroke or transient ischemic attack (TIA). In people 75 and older it is generally not recommended, because of a higher risk of fatal and intracranial bleeding with uncertain benefit, though a doctor may still consider it in high-risk situations such as diabetes or a prior heart attack. It should not be started in people likely to need urgent bypass surgery (CABG), and when surgery is planned it should be stopped at least 7 days ahead when possible. Other things that raise bleeding risk include body weight under 60 kg (a lower daily maintenance dose is recommended), a tendency to bleed, and taking anticoagulants, NSAIDs, or clot-dissolving drugs at the same time.
Other warnings include thrombotic thrombocytopenic purpura (TTP), which can appear within the first two weeks, and hypersensitivity reactions including angioedema. Common side effects are easy bruising, nosebleeds, other minor bleeding, high blood pressure, headache, dizziness, back pain, and nausea. Do not stop prasugrel early on your own — stopping without cardiology guidance raises the risk of stent thrombosis, heart attack, and death.
